2009 AMCP/FMCP/Pfizer Inc. Managed Care Pharmacy Summer Internship Program

**Please Note** The following three letters are also required. Each letter must be written by a different individual and must be received in its own sealed envelope. All three letters must be mailed in one package and sent to: AMCP/FMCP/Pfizer Inc. Summer Internship Coordinator c/o FMCP, 100 North Pitt Street, Suite 400, Alexandria, VA 22314. All letters must be postmarked by February 2, 2009.
