FMCP Seeks Nominees for the 2011 Steven G. Avey Award
The Foundation is seeking nominations for the 2011 Steven G. Avey Award to be presented to the recipient during the Second Annual Steven G. Avey Award Lecture at AMCP's 23rd. Annual Meeting & Showcase in Minneapolis, MN April 27-30, 2011. The Awards Committee is seeking nominations of healthcare professionals who have shown great leadership, vision and dedication to managed care pharmacy.
Who Does This Award Recognize?
The Steven G. Avey Award recognizes an individual for sustained, exemplary, and distinguished service to the practice of managed care pharmacy.
Who Can Nominate Individuals for This Award?
Anyone may nominate a candidate or themselves for this award. Nominees may also be asked for qualifying information. For each nomination, there must be a nominator plus five individual supporters, for a total of six references, providing information about the nominee’s sustained, exemplary and distinguished service to the field of managed care pharmacy.
Who Can Be Nominated for This Award?
Candidates should be healthcare professional role models who have made significant and sustained contributions to the advancement of the field of managed care pharmacy in clinical and/or administrative practice. Candidates are not required to be AMCP members. Current AMCP Board members, FMCP Trustees, or employees of either organization are not eligible. The award may be given posthumously.

Nominations must be accompanied by the Steven G. Avey Award Nominations Form, the nominee’s CV/résumé and the 500-word Nominee Background Information statement. Nominations may also be submitted online by following the link below.

Who Will Select the Award Recipient?
The Steven G. Avey Award selection committee, appointed by FMCP, will select the Award recipient.
What Will Be Awarded?
The Steven G. Avey Award recipient will be announced at the Second Annual Steven G. Avey Lecture held during AMCP’s 23rd Annual Meeting & Showcase in Minneapolis, MN April 27-30, 2011. The recipient will receive the engraved Steven G. Avey Award crystal commemorative and have his or her name engraved on a plaque permanently displayed at the AMCP and FMCP national headquarters. FMCP will provide reimbursement for reasonable travel expenses associated with the 23rd Annual Meeting & Showcase.
